Dear friends and followers, welcome back to my channel!
On the 20th of February 2021, a United Boeing 777-200, registration N772UA performing flight UA-328 from Denver,CO to Honolulu,HI (USA) with 231 passengers and 10 crew, was in the initial climb out of Denver's runway 25 when the right hand engine's (PW4077) inlet separated associated with the failure of the engine. The crew declared Mayday reporting an engine failure. The aircraft stopped the climb at about 13000 feet, the crew requested to return to Denver after running the checklists. ATC offered any runway, they would make it happen. The aircraft returned to Denver for a safe landing on runway 26 about 23 minutes after departure. The aircraft stopped on the runway for a check by emergency services. Emergency services advised of an active fire within the right hand engine and extinguished the fire a few minutes later. The aircraft was subsequently towed off the runway to a remote parking stand, where passengers disembarked and were bussed to the terminal. There were no injuries.
The engine inlet fell into the neighbourhood of Broomfield,CO, located about 16nm west of Denver near 13th and Elmwood Street, the debris also struck through the roof of an adjacent house.
Broomfield police reported that although debris impacted the neighbourhood and damaged a number of homes, there were no injuries on the ground. The debris field expands over a nautical mile.
Ground observers reported hearing the sound of an explosion like bang, smoke and saw the debris falling down. The aircraft continued flying.

Why the engine failed is under investigation. It won't take long for that since there's data out the ying yang streaming out of both engine, and the engine itself still there on the wing. The checklist part is SOP and most likely engine failure or fire in flight, engine shut down, and landing with one engine inoperative. Standard stuff and always practiced in the simulator every six months with US air carriers. Even tough this kind of failure is rare, the procedures are routine, and the outcome was as expected. Great job crew! Didn't need some phony ILfor Captain wearing co-pilot shoulder boards (who's a legend in his own mind) to waste time on this one.

shouldnt there be a 'brake' option for the turbine in such instances where there remains a hi risk of further degradation due to impeller inbalance, having lost a blade? - Seems unwise for manufacturer to allow a damaged turbine to free wheel or worse following damage

I've seen a video from a turbine engine expert that raises a few points about the engine itself that even Captain Joe may not be completely right on: 1. The engine failure was contained. The only parts falling off were components of the nacelle, which are aircraft parts, not engine parts. The engine casing itself, which incorporates a kevlar containment layer, is totally intact. This is the brown jacket that can be seen at the front of the engine in the video. This is an actual engine part, the inlet around it is an aircraft part. 2. The fire on the engine looks scary, but it's actually quite superficial. It occurs around the (closed) thrust reverser duct, which is the grille pattern that you see engulfed in the flames toward the rear of the engine nacelle. This is on the outside of the fan duct, quite far away from the actual high-pressure / low-pressure turbine stages in the engine core. He speculates that it may be simply an oil fire from leaking oil lines. In other words, all of the safety features on the engine totally contained the failure. The detached fan blade did not puncture any exterior parts of the aircraft, and the kevlar containment jacket was intact. The resulting fan imbalance would have created massive vibrations, seen even as the engine continues windmilling, and this effect would have been strong enough literally to shake off the engine nacelle.

Excellent presentation, Pilot Joe! I have been wondering to get one of the answers about the checklist of Maximum Landing Weight??? Apparently it was a 7 hour flight, with at least 110 tons of trip fuel, 231 passengers, their baggages and cargo on board. Was the aircraft still within the maximum landing weight limits??? So, I will be interested to know that Checklist for sure that you leave it for Authorities to respond. Thanks for the video posted!

Luckily the disintegration of shroud parts didn’t randomly fly back into tail and wing structures. Real possibility of flight control damage and resultant loss of aircraft axis control. Explosive disintegration of fan blade assembly due to fatigue cracks in airfoil components. Centrifugal force upon rotating components causing immediate shroud damage. Avoid if possible turning into dead wing. Possible stall conditions of damaged wing exacerbated by increasing lift regime of remaining good engine. I’ve always believed the tri engine design affording a safety design margin. Manipulation of remaining engines to lessen roll and pitch anomalies. Near misses are warnings. Effect an immediate inspection protocol of concerned engines of fleet.

The crew did a great job, and Joe does a great job of pointing that out here. Are you sure it was an uncontained failure? I love it when a pilot tries to explain something that needs a mechanic. Don't want to be mean Joe, but try and get close, sorry Joe couldn't let it slide. The fire retardant, is a fire extinguishing agent, not detergent, were not cleaning anything here, and it is not injected into the turbines, it is injected into the engine cowling around the engine. The reason why the fire extinguishing agent probably didn't work was due to all the missing cowling, the fire retardant displaces the oxygen.

This was a contained failure. (almost)100% very little of the engine will be found on the ground..Those parts found are aerodynamic parts of the aircraft AROUND the engine - cowlings and so on.
